The adults in our home have Individual service plans and Individual

health care plans that focus on personalized goals.

Occasionally families and caregivers may not have the capacity to provide for family members or friends with challenging physical, medical, or behavioral needs. That’s when individuals with intellectual and developmental disabilities and their families turn to the resources provided in the community-based specialized residential support services. Baycare House works with Inland Regional Center to ensure that our facility is tailored to fit the needs of everyone who needs support with certain daily activities that require 24/7 care. Accordingly, the family members and friends would then rest assured that their loved ones are receiving appropriate support and compassionate residential care in the facility.

The facility offers the following specialized health services that support up to four individuals

  • Respiratory therapy
  • Nutritional support
  • Special medication regimes
  • Community integration

Baycare House offers residential care and support for adults with developmental disabilities within the Inland Regional Center catchment area. The facility ensures individuals feel comfortable, safe, and supported. Baycare House enables individuals to receive the support they need in the shared space including

  • Personalized support to help individuals become more independent.
  • Specialized services for individuals who have physical or behavioral challenges.
  • 24/7 treatment services for people with special medical needs.

Personalized Care and Community Integration at Baycare

The staff observes and evaluates each person’s needs and develops Individualized Service Plans that meet the needs of everyone, in conjunction with support from a physician and a psychiatrist.
Further, individuals are encouraged to be a part of the community. Regardless of the intellectual or developmental disability, Baycare staff accompanies individuals on walks, trips to nearby cities and landmarks, and other community events.

Service Description

Successful Placement

Careful evaluation of an individual’s needs based on social and personal interests, and medical and vocational needs, forms the first criteria of placement. The facility staff team works together with everyone, their family, and professionals at the Inland Regional Center to create successful matches and develop Individual Service Plans (ISP) for each person we support.

Individualized Support

Often behavior challenges occur when a person feels not heard or ignored. As such, when Individuals participate in the activities suited their needs and preferences behavior challenges are mitigated and minimized. The facility ensures every individual in the facility enjoy learning, making new friends, as well as reach the milestones that are important to them. Baycare staff ensures that the goals set in the Individualized Service Plans (ISP) are implemented and recorded

A facility is more than just a shared space. It offers a base in which we build meaningful connections and share our lives with the people we care about. Baycare House helps individuals with intellectual and developmental disabilities live in private, within a quiet neighborhood in Moreno Valley.

In conjunction with our Behavior Specialist, the facility considers and enhances the interests, skills, and personalities of individuals to create a good match with individuals that lead to meaningful, long-term relationships.
Baycare House provides individuals with personalized services and countless opportunities to take part in everyday activities of family life, such as:

Day Programs

Contracting with the day programs facilities enables individuals in our care, to learn, grow, and discover new possibilities in the communities and neighborhood.  The individual learns to develop new interests and new goals, interact with others, and participate in meaningful activities. The local day programs help individuals reach goals, among them:

  • Learning vocational skills
  • Improving independent living skills
  • Gaining paid employment
  • Earning a college degree

Meaningful Activities

Center-based Programs: Feature structured, meaningful activities designed to develop new skills at a location where participants look forward to seeing friends and staff members each day.
Community-based Programs: Give individuals the chance to get out in the community for engaging experiences involving employment, volunteering, and recreational activities.
Activity and Leisure Program (PAL): Brings care and goal-driven support to the homes of adults who are unable to attend a traditional day program.
